What Is Adderall XR 20 mg?

Adderall XR 20 mg is a mid-strength capsule of the extended-release (XR) formulation of mixed amphetamine salts. It is a central nervous system (CNS) stimulant classified as a Schedule II controlled substance in the United States due to its high potential for abuse and dependence.

The 20 mg strength is one of the most commonly prescribed doses of Adderall XR. It is frequently used as a maintenance dose for adults and adolescents who have been titrated up from lower strengths, and it provides approximately 10 to 12 hours of symptom coverage with once-daily morning dosing.

What Adderall XR 20 mg Is Prescribed For

Adderall XR 20 mg is FDA-approved for one condition:

  • Attention-Deficit/Hyperactivity Disorder (ADHD) — in adults and pediatric patients 6 years and older.

Important limitation: Adderall XR is not recommended for children under 6 years of age because they have higher plasma exposure and a higher incidence of adverse reactions (such as weight loss) at the same dosage compared to patients 6 years and older.

Note on narcolepsy: Unlike immediate-release Adderall, the extended-release XR formulation is not FDA-approved for narcolepsy. Only the immediate-release tablet is approved for that condition.

Active Ingredients

The active ingredients in Adderall XR 20 mg are the same four amphetamine salts found in the immediate-release formulation:

  • Dextroamphetamine saccharate
  • Amphetamine aspartate
  • Dextroamphetamine sulfate
  • Amphetamine sulfate

These salts are present in a 3:1 ratio of d-amphetamine to l-amphetamine.

The 20 mg capsule is described as orange/orange in color.

Immediate-Release vs. Extended-Release

Adderall XR 20 mg is an extended-release capsule. It should not be confused with Adderall immediate-release (IR) tablets, which have different pharmacokinetics and dosing.

Feature Immediate-Release (IR) Tablets Extended-Release (XR) Capsules Available strengths 5 mg, 7.5 mg, 10 mg, 12.5 mg, 15 mg, 20 mg, 30 mg 5 mg, 10 mg, 15 mg, 20 mg, 25 mg, 30 mg Onset (Tmax) ~3 hours ~7 hours Duration Approximately 4–6 hours Approximately 10–12 hours Dosing frequency Often multiple times daily Once daily, typically in the morning Minimum age (ADHD) 3 years 6 years Narcolepsy indication Yes NoKey pharmacokinetic data: A single dose of Adderall XR 20 mg provides comparable plasma concentration profiles to Adderall IR 10 mg taken twice daily, 4 hours apart. This means the 20 mg XR capsule delivers the equivalent of two 10 mg IR doses spread across the day.

Administration note: The capsule may be swallowed whole, or opened and the contents sprinkled on applesauce. The sprinkled applesauce should be consumed immediately without chewing; it should not be stored.

Important Safety Information (Boxed Warning)

Abuse, Misuse, and Addiction: Adderall XR 20 mg has a high potential for abuse and misuse, which can lead to the development of a substance use disorder, including addiction. Misuse and abuse of CNS stimulants, including Adderall XR, can result in overdose and death. This risk is increased with higher doses or unapproved methods of administration, such as snorting or injection.

Before prescribing Adderall XR, healthcare providers should assess each patient’s risk for abuse, misuse, and addiction. Throughout treatment, risk should be reassessed and patients monitored for signs and symptoms of abuse, misuse, and addiction.

Adderall XR may lead to physical dependence after prolonged use, even if taken as directed. Do not give Adderall XR to anyone else. Selling or giving away this medication is against the law and may harm others.

Common Side Effects

Pediatric patients ages 6 to 12: Loss of appetite, insomnia, abdominal pain, emotional lability, vomiting, nervousness, nausea, and fever.

Pediatric patients ages 13 to 17: Loss of appetite, insomnia, abdominal pain, weight loss, and nervousness.

Adults: Dry mouth, loss of appetite, insomnia, headache, weight loss, nausea, anxiety, agitation, dizziness, tachycardia, diarrhea, asthenia, and urinary tract infections.

Weight change note: In a controlled trial of Adderall XR in adolescents, mean weight change from baseline within the initial 4 weeks of therapy was –1.1 lbs for patients receiving 10 mg and –2.8 lbs for those receiving 20 mg. Higher doses were associated with greater weight loss within the initial 4 weeks of treatment.

Important Warnings

Cardiovascular Risks: Sudden death has been reported in patients with structural cardiac abnormalities or other serious heart disease who were treated with CNS stimulants at recommended ADHD dosages. Adderall XR should be avoided in patients with known structural cardiac abnormalities, cardiomyopathy, serious cardiac arrhythmia, coronary artery disease, or other serious cardiac disease. Adderall causes modest increases in average blood pressure and heart rate. Blood pressure and pulse should be monitored at appropriate intervals.

Psychiatric Risks: Stimulants may cause new or worsening mental problems, including psychosis, mania, or bipolar illness. Treatment-emergent psychotic or manic symptoms (such as hallucinations, delusional thinking, or mania) can occur in patients without prior history of psychotic illness. In a pooled analysis of multiple short-term, placebo-controlled studies, such symptoms occurred in about 0.1% of stimulant-treated patients compared to 0 in placebo-treated patients. Aggressive behavior or hostility has also been reported.

Long-Term Growth Suppression: CNS stimulants have been associated with weight loss and a slowing of growth rate in children. Careful follow-up of children ages 7 to 10 suggests consistently medicated children may have a temporary slowing in growth rate (on average, about 2 cm less growth in height and 2.7 kg less growth in weight over 3 years). Height and weight should be monitored during treatment.

Peripheral Vasculopathy (Raynaud’s Phenomenon): Stimulants are associated with circulation problems in fingers and toes. Signs include fingers or toes feeling numb, cool, or painful, or changing color from pale to blue to red. Further clinical evaluation (e.g., rheumatology referral) may be appropriate for certain patients.

Serotonin Syndrome: A potentially life-threatening reaction that may occur when Adderall XR is taken with other serotonergic drugs. Symptoms include mental status changes, agitation, hallucinations, fast heartbeat, flushing, seizures, loss of coordination, confusion, dizziness, changes in blood pressure, sweating or fever, nausea, vomiting, diarrhea, muscle stiffness, and high body temperature. Stop taking Adderall XR and seek emergency care immediately if these occur.

Seizures: Stimulants may lower the convulsive threshold. Adderall XR should be discontinued if seizures occur.

New or Worsening Tics: Adderall XR may cause or worsen motor and verbal tics, or worsen Tourette’s syndrome. Before initiating treatment, assess family history and clinically evaluate patients for tics or Tourette’s syndrome.

Drug Interactions

MAOIs (Monoamine Oxidase Inhibitors): Taking Adderall XR with MAOIs, or within 14 days of stopping an MAOI, can cause a hypertensive crisis with potentially fatal outcomes. This combination is contraindicated.

Serotonergic Drugs: Increased risk of serotonin syndrome. Examples include SSRIs, SNRIs, triptans, tricyclic antidepressants, fentanyl, lithium, tramadol, tryptophan, buspirone, and St. John’s Wort.

CYP2D6 Inhibitors: May increase amphetamine exposure and risk of serotonin syndrome. Examples include paroxetine, fluoxetine, quinidine, and ritonavir.

Alkalinizing Agents (e.g., antacids, sodium bicarbonate): Increase blood levels and potentiate the effects of amphetamine. Co-administration with Adderall XR should be avoided.

Acidifying Agents (e.g., ascorbic acid, fruit juices): Lower blood levels and reduce efficacy of amphetamines.

Who Should Not Take Adderall XR 20 mg

Adderall XR is contraindicated in patients with:

  • Known hypersensitivity to amphetamine or other components of Adderall XR. Hypersensitivity reactions such as angioedema and anaphylaxis have been reported.
  • Current or recent (within 14 days) use of MAOIs.

How Prescriptions Work

As a Schedule II controlled substance, Adderall XR 20 mg prescriptions are subject to strict federal and state regulations:

  • No refills allowed: Federal law prohibits refills on Schedule II prescriptions. A new prescription is required each time.
  • Limited quantity: Many jurisdictions limit the quantity that can be prescribed at once (often a 30-day supply).
  • Regular follow-up: Patients may need regular follow-up visits before receiving a new prescription.
  • Lowest effective dose: Prescribers should individualize dosage and use the lowest effective dose.
  • Maximum daily doses: The maximum recommended dosage is 60 mg/day for adults, 40 mg/day for adolescents (13–17 years), and 30 mg/day for children (6–12 years).
  • Renal impairment: The initial dosage should be decreased in people with severe kidney disease to 5 mg in children and 15 mg in adults.
  • Safe storage: Store Adderall XR at room temperature (20° to 25°C / 68° to 77°F), protect from light, and keep in a safe place such as a locked cabinet. Schedule II substances should be securely stored, ideally in a locked container.

Frequently Asked Questions

Is Adderall XR 20 mg safe for long-term use? The effectiveness of Adderall XR for long-term use has not been systematically evaluated in controlled trials. Physicians should periodically re-evaluate the long-term usefulness for each patient.

Does Adderall XR 20 mg affect sleep? Insomnia is a common side effect. Taking the medication earlier in the morning can help minimize sleep difficulties. Adderall XR should not be taken in the afternoon or evening.

Can I stop taking Adderall XR 20 mg suddenly? Abrupt discontinuation after prolonged use can lead to withdrawal symptoms, including extreme fatigue and mental depression. This should be managed under medical supervision.

What does “20 mg” mean in Adderall XR 20 mg? The 20 mg refers to the total amount of the four amphetamine salts combined. It is not two separate 20 mg doses—it is a single 20 mg dose released over time.

Is Adderall XR 20 mg the same as Adderall IR 20 mg? No. Adderall XR 20 mg is an extended-release capsule, while Adderall IR 20 mg is an immediate-release tablet. They have different onset times, durations, and dosing schedules. Adderall XR 20 mg provides roughly comparable total daily exposure to Adderall IR 10 mg taken twice daily.

Can Adderall XR 20 mg be opened and sprinkled? Yes. The capsule may be opened and the contents sprinkled on applesauce. This results in comparable absorption to swallowing the capsule whole. The sprinkled mixture should be consumed immediately and not stored.

What is the starting dose of Adderall XR for children? The usual starting dose for children ages 6 to 12 is 5–10 mg once daily in the morning. The dose may be increased in 5–10 mg increments at weekly intervals up to a maximum of 30 mg/day.

Conclusion

Adderall XR 20 mg is a mid-strength extended-release stimulant approved for ADHD in patients 6 years and older. As a Schedule II controlled substance, it carries a boxed warning for abuse, misuse, and addiction, and requires strict prescribing controls. Common side effects include appetite loss, insomnia, and dry mouth. Serious risks include cardiovascular events, psychiatric symptoms, growth suppression in children, and serotonin syndrome when combined with serotonergic drugs. The capsule may be swallowed whole or sprinkled on applesauce. Patients should use the lowest effective dose, monitor height and weight (in children), store the medication securely, and never share it with others. Any changes to treatment should be managed under medical supervision.
