What Is Adderall XR 5 mg?

Adderall XR 5 mg is the lowest available strength of the extended-release (XR) capsule formulation of mixed amphetamine salts. It is a central nervous system (CNS) stimulant classified as a Schedule II controlled substance in the United States due to its high potential for abuse and dependence.

The 5 mg strength is the minimum single-capsule dose approved for ADHD. It is typically used as a conservative starting dose for patients who are sensitive to stimulant medication, for those beginning treatment at a lower initial dose, or for children transitioning from immediate-release formulations at equivalent total daily exposure.

What Adderall XR 5 mg Is Prescribed For

Adderall XR 5 mg is FDA-approved for one condition:

  • Attention-Deficit/Hyperactivity Disorder (ADHD) — in adults and pediatric patients 6 years and older.

Important limitation: Adderall XR is not recommended for children under 6 years of age because they have higher plasma exposure and a higher incidence of adverse reactions (such as weight loss) at the same dosage compared to patients 6 years and older.

Note on narcolepsy: Unlike immediate-release Adderall, the extended-release XR formulation is not FDA-approved for narcolepsy. Only the immediate-release tablet is approved for that condition.

Active Ingredients

The active ingredients in Adderall XR 5 mg are the same four amphetamine salts found in all Adderall formulations, each present at 1.25 mg per capsule:

  • Dextroamphetamine saccharate — 1.25 mg
  • Amphetamine aspartate monohydrate — 1.25 mg
  • Dextroamphetamine sulfate — 1.25 mg
  • Amphetamine sulfate — 1.25 mg

The total amphetamine base equivalence is 3.1 mg per 5 mg capsule, with 2.4 mg d-amphetamine base and 0.75 mg l-amphetamine base.

The 5 mg capsule is described as clear/blue in color and imprinted with “ADDERALL XR 5 mg.”

Immediate-Release vs. Extended-Release

Adderall XR 5 mg is an extended-release capsule. It should not be confused with Adderall immediate-release (IR) tablets, which have different pharmacokinetics and dosing.

Feature Immediate-Release (IR) Tablets Extended-Release (XR) Capsules Available strengths 5 mg, 7.5 mg, 10 mg, 12.5 mg, 15 mg, 20 mg, 30 mg 5 mg, 10 mg, 15 mg, 20 mg, 25 mg, 30 mg Onset (Tmax) ~3 hours ~7 hours Duration Approximately 4–6 hours Approximately 10–12 hours Dosing frequency Often multiple times daily Once daily, typically in the morning Minimum age (ADHD) 3 years 6 years Narcolepsy indication Yes NoKey pharmacokinetic data: A single dose of Adderall XR 20 mg provides comparable plasma concentration profiles to Adderall IR 10 mg taken twice daily, 4 hours apart. This means Adderall XR 5 mg is roughly comparable in total daily exposure to Adderall IR 2.5 mg taken twice daily.

Administration note: The capsule may be swallowed whole, or opened and the contents sprinkled on applesauce. The sprinkled applesauce should be consumed immediately without chewing; it should not be stored. The dose of a single capsule should not be divided.

Important Safety Information (Boxed Warning)

Abuse, Misuse, and Addiction: Adderall XR 5 mg has a high potential for abuse and misuse, which can lead to the development of a substance use disorder, including addiction. Misuse and abuse of CNS stimulants, including Adderall XR, can result in overdose and death. This risk is increased with higher doses or unapproved methods of administration, such as snorting or injection.

Before prescribing Adderall XR, healthcare providers should assess each patient’s risk for abuse, misuse, and addiction. Throughout treatment, risk should be reassessed and patients monitored for signs and symptoms of abuse, misuse, and addiction.

Adderall XR may lead to physical dependence after prolonged use, even if taken as directed. Do not give Adderall XR to anyone else. Selling or giving away this medication is against the law and may harm others.

Common Side Effects

Pediatric patients ages 6 to 12: Loss of appetite, insomnia, abdominal pain, emotional lability, vomiting, nervousness, nausea, and fever.

Pediatric patients ages 13 to 17: Loss of appetite, insomnia, abdominal pain, weight loss, and nervousness.

Adults: Dry mouth, loss of appetite, insomnia, headache, weight loss, nausea, anxiety, agitation, dizziness, tachycardia, diarrhea, asthenia, and urinary tract infections.

Weight change note: In a controlled trial of Adderall XR in adolescents, mean weight change from baseline within the initial 4 weeks of therapy was –1.1 lbs for patients receiving 10 mg and –2.8 lbs for those receiving 20 mg. Higher doses were associated with greater weight loss within the initial 4 weeks of treatment.

Important Warnings

Cardiovascular Risks: Sudden death has been reported in patients with structural cardiac abnormalities or other serious heart disease who were treated with CNS stimulants at recommended ADHD dosages. Adderall XR should be avoided in patients with known structural cardiac abnormalities, cardiomyopathy, serious cardiac arrhythmia, coronary artery disease, or other serious cardiac disease. Adderall causes modest increases in average blood pressure (about 2–4 mmHg) and heart rate (about 3–6 bpm).

Psychiatric Risks: Stimulants may cause new or worsening mental problems, including psychosis, mania, or bipolar illness. Treatment-emergent psychotic or manic symptoms (such as hallucinations, delusional thinking, or mania) can occur in patients without prior history of psychotic illness. In a pooled analysis of multiple short-term, placebo-controlled studies, such symptoms occurred in about 0.1% of stimulant-treated patients compared to 0 in placebo-treated patients.

Long-Term Growth Suppression: CNS stimulants have been associated with weight loss and a slowing of growth rate in children. Careful follow-up of children ages 7 to 10 suggests consistently medicated children may have a temporary slowing in growth rate (on average, about 2 cm less growth in height and 2.7 kg less growth in weight over 3 years). Height and weight should be monitored during treatment.

Peripheral Vasculopathy (Raynaud’s Phenomenon): Stimulants are associated with circulation problems in fingers and toes. Signs include fingers or toes feeling numb, cool, or painful, or changing color from pale to blue to red. Further clinical evaluation (e.g., rheumatology referral) may be appropriate for certain patients.

Serotonin Syndrome: A potentially life-threatening reaction that may occur when Adderall XR is taken with other serotonergic drugs. Symptoms include mental status changes, agitation, hallucinations, fast heartbeat, flushing, seizures, loss of coordination, confusion, dizziness, changes in blood pressure, sweating or fever, nausea, vomiting, diarrhea, muscle stiffness, and high body temperature. Stop taking Adderall XR and seek emergency care immediately if these occur.

Seizures: Stimulants may lower the convulsive threshold. Adderall XR should be discontinued if seizures occur.

New or Worsening Tics: Adderall XR may cause or worsen motor and verbal tics, or worsen Tourette’s syndrome. Before initiating treatment, assess family history and clinically evaluate patients for tics or Tourette’s syndrome.

Drug Interactions

MAOIs (Monoamine Oxidase Inhibitors): Taking Adderall XR with MAOIs, or within 14 days of stopping an MAOI, can cause a hypertensive crisis with potentially fatal outcomes. This combination is contraindicated.

Serotonergic Drugs: Increased risk of serotonin syndrome. Examples include SSRIs, SNRIs, triptans, tricyclic antidepressants, fentanyl, lithium, tramadol, tryptophan, buspirone, and St. John’s Wort.

CYP2D6 Inhibitors: May increase amphetamine exposure and risk of serotonin syndrome. Examples include paroxetine, fluoxetine, quinidine, and ritonavir.

Alkalinizing Agents (e.g., antacids, sodium bicarbonate): Increase blood levels and potentiate the effects of amphetamine. Co-administration with Adderall XR should be avoided.

Acidifying Agents (e.g., ascorbic acid, fruit juices): Lower blood levels and reduce efficacy of amphetamines.

Who Should Not Take Adderall XR 5 mg

Adderall XR is contraindicated in patients with:

  • Known hypersensitivity to amphetamine or other components of Adderall XR. Hypersensitivity reactions such as angioedema and anaphylaxis have been reported.
  • Advanced arteriosclerosis
  • Symptomatic cardiovascular disease
  • Moderate to severe hypertension
  • Hyperthyroidism
  • Glaucoma
  • Agitated states
  • History of drug abuse
  • Current or recent (within 14 days) use of MAOIs

How Prescriptions Work

As a Schedule II controlled substance, Adderall XR 5 mg prescriptions are subject to strict federal and state regulations:

  • No refills allowed: Federal law prohibits refills on Schedule II prescriptions. A new prescription is required each time.
  • Limited quantity: Many jurisdictions limit the quantity that can be prescribed at once (often a 30-day supply).
  • Regular follow-up: Patients may need regular follow-up visits before receiving a new prescription.
  • Lowest effective dose: Prescribers should individualize dosage and use the lowest effective dose.
  • Maximum daily doses: The maximum recommended dosage is 60 mg/day for adults, 40 mg/day for adolescents (13–17 years), and 30 mg/day for children (6–12 years).
  • Safe storage: Store Adderall XR at room temperature (20° to 25°C / 68° to 77°F), protect from light, and keep in a safe place such as a locked cabinet. Schedule II substances should be securely stored, ideally in a locked container.

Frequently Asked Questions

Is Adderall XR 5 mg safe for long-term use? The effectiveness of Adderall XR for long-term use has not been systematically evaluated in controlled trials. The physician who elects to use Adderall XR for extended periods should periodically re-evaluate the long-term usefulness of the drug for the individual patient.

Does Adderall XR 5 mg affect sleep? Insomnia is a common side effect. Adderall XR should be given upon awakening. Afternoon doses should be avoided because of the potential for insomnia.

Can I stop taking Adderall XR 5 mg suddenly? Abrupt discontinuation after prolonged use can lead to withdrawal symptoms, including extreme fatigue and mental depression. This should be managed under medical supervision.

What does “5 mg” mean in Adderall XR 5 mg? The 5 mg refers to the total amount of the four amphetamine salts combined. Each of the four salts is present at 1.25 mg per capsule. The total amphetamine base equivalence is 3.1 mg.

Is Adderall XR 5 mg the same as Adderall IR 5 mg? No. Adderall XR 5 mg is an extended-release capsule, while Adderall IR 5 mg is an immediate-release tablet. Adderall XR 5 mg provides roughly comparable total daily exposure to Adderall IR 2.5 mg taken twice daily.

Can Adderall XR 5 mg be opened and sprinkled? Yes. The capsule may be opened and the contents sprinkled on applesauce. This results in comparable absorption to swallowing the capsule whole. The sprinkled mixture should be consumed immediately and not stored.

What is the starting dose of Adderall XR for children? The usual starting dose for children ages 6 to 12 is 5–10 mg once daily in the morning. When in the judgment of the clinician a lower initial dose is appropriate, patients may begin treatment with 5 mg once daily in the morning.

Conclusion

Adderall XR 5 mg is the lowest-strength extended-release stimulant approved for ADHD in patients 6 years and older. As a Schedule II controlled substance, it carries a boxed warning for abuse, misuse, and addiction, and requires strict prescribing controls. Common side effects include appetite loss, insomnia, and abdominal pain. Serious risks include cardiovascular events, psychiatric symptoms, growth suppression in children, and serotonin syndrome when combined with serotonergic drugs. The capsule may be swallowed whole or sprinkled on applesauce. Patients should use the lowest effective dose, monitor height and weight (in children), store the medication securely, and never share it with others. Any changes to treatment should be managed under medical supervision.
